Epistaxis

Some degree of epistaxis is expected, especially since routine nasal packing is no longer employed. Delayed epistaxis, occurring 2–3 weeks postoperative, is most commonly from a branch of the sphenopalatine artery and responds to packing or endoscopic cauterization. Severe bleeding, especially intraoperatively, should be evaluated with urgent carotid angiography or CT angiography to exclude a pseudoaneurysm in the cavernous carotid artery [47].

Infection

Chronic sphenoid sinusitis may occur in up to 10% cases following transsphenoidal surgery [133] and usually responds to antibiotics, although repeat cases may require eventual operative drainage [134]. Meningitis is rare, but is usually seen in association with an undetected CSF leak.
